CRP Presents: Siloam Family Health Center Community Health Outreach Program – Oct. 7

NASHVILLE, Tenn. Siloam Family Health Center is a primary and specialty care clinic that serves uninsured patients. About 90% are foreign-born, representing more than 80 homelands and speaking more than 70 languages. Our patients tell us that their number one need is help navigating the confusing world of American healthcare. Patients want people they can trust to walk alongside them, explain things on their terms, and help them recover a sense of agency to take charge of their own health and well-being. Drawing upon lessons learned from effective health care projects among poor and marginalized communities globally, Siloam’s Community Health Outreach (CHO) program is training and deploying a cadre of Community Health Workers (CHWs) to create a collaborative support network within four communities – Bhutanese Burmese, Egyptian, and Hispanic – using ethnic churches as bases. We will share our CHO model, our training process, and progress to date.

About the Meharry-Vanderbilt Alliance

Founded in 1999, the Alliance bridges the institutions of Meharry Medical College and Vanderbilt University Medical Center. Its mission is to enrich learning and advance clinical research in three primary areas -- community engagement, interprofessional education and research -- by developing and supporting mutually beneficial partnerships between Meharry Medical College, Vanderbilt University Medical Center and the communities they serve. Through community engagement, the Alliance serves a large community of stakeholders including surrounding universities and colleges, community organizations, faith-based outlets and community health centers. Its interprofessional education enhances students' interdisciplinary understanding and improves patient outcomes through integrated care. The research conducted provides access to experienced grant writers and materials supporting the grant application process and facilitates grant-writing workshops.
