For nearly 10 years, our nationally recognized historic collaboration has enjoyed many sentinel accomplishments.

At our recent retreat, we set forth to design and create a “Revolution in Healthcare” that will implement a novel system of care for a specific medical condition applicable to a defined population within Davidson County and/or middle Tennessee.

In order to accomplish this lofty and evolutionary vision, we need to collaborate and develop such a model that will also be portable and scalable.

We are announcing an opportunity across Alliance partners to compete for a planning grant to evolve ideas into action.  Here is a link to the program overview and application form for you to submit for competitive review.  Funding will be available through the Alliance Foundation and/or other identified resources.
